The DEA requires a copy of your current Federal DEA registration certificate to be on file prior to shipping controlled substances. The address on the DEA license must match the shipping address. Fax a copy of your DEA license to 800-876-7621.
Federal Law requires a properly completed DEA From 222 (See below instructions and sample) for Class II drugs. Shipment of Class II drugs cannot be made until Form 222 has been received. Improperly completed forms or forms with alterations can not be processed.
